Student Aid Form for SY 12-13

Families who wish to apply for Diocesan Grant for SY 12-13 are required to mail a completed PSAS application (attached and also available in the school office) no later then March 30, 2012 to Private School Aid Service, P.O. BOX 89434, Cleveland , OH 44101-6434. Please be advised in order to be considered to receive a Parish Grant, you are required to submit a PSAS application.
